CPR Financial Assistance
CPR provides a sliding Fee Discount Program (SFDP) based on family size and income. If you would like to apply for this program, the following steps need to be completed in order for your application to be considered. Please note, applying does not guarantee approval.
No one will be denied access to services due to inability to pay.

STEP 2: Complete the required information on the application
STEP 3: Submit the application to CPR before your next appointment (see submittal options below):

CPR will review the application and if you qualify for the sliding fee discount, a CPR representative will contact you to discuss the steps required to complete the process. Verification of identification and income is required. This includes submittal of a valid ID, pay stubs, tax returns and other forms upon request. Please see the application for more details.
