Transitional Care

The CPR Transitional Care Program helps bridge service gaps between different levels and types of care, addressing issues like limited support after hospital discharge and difficulty accessing outpatient services. The program aims to support clients during transition periods, focusing on short term clinical engagement and education regarding benefits of primary care / support. The program is available to adults aged 18 and up, and accepts most insurance plans including AHCCCS. Clients can connect with  counselors, psychiatric nurse practitioners, medical assistants and case managers in person or virtually at several touchpoints during their transitional period. This helps prevent unnecessary hospital readmissions and reduces the negative financial impact on insurance plans, hospitals and clients.
