To Cancel an Appointment

When leaving a voicemail or sending an email to cancel an appointment, please provide your full name, date of birth, the date and time of the appointment you would like to cancel, and a reason for the cancellation.

Please note that failing to cancel or show up for appointments with less than 24 hours' notice will result in a fee of $150 for initial appointments and $75 for follow-up appointments. These fees are not covered by insurance.

Request Records

Please be advised that we require photo identification to provide medical records. When completing the online record request form, please send a copy of your photo identification to the above email or fax so that the Record Requests department can process your request as efficiently as possible. If the Record Requests department is unable to obtain a copy of your identification, you will be asked to provide a notarized signature.
